G.A.P. is not a one time referral. It's a 12 month relationship built around your goals with real accountability, resources, and results.

Complete your intake form
A 5-minute form that tells us about your goals and current situation. No judgment. No barriers. Just your story.

Your CSM Gets In Contact
A dedicated Case Success Manager contacts you within 3–5 business days to complete your application and answer every question you have.

Build your Game Plan
Together, you and your CSM create 12 personalized monthly milestones towards your education, career, or mental health goal.

Work toward your goals
Monthly checkins, barrier removal, resource access, and community workshops keep you on track for the full 12 months and beyond.
